latest theory I am seeing is that something went awry withe the 787's Thrust Control Malfunction Accommodation mechanism.  It is supposed to address runaway thrust increases on engines.  The thinking it is that inappropriately engaged and doomed that Air India flight.  Lord knows Boeing has had a number of software issues.  That 737 Max debacle and a few for the 787:

More 737-MAX woes.........   Engine ingests a bird.  Some sort of gizmo engages to try and keep the engine from melting down but that gizmo also seems to gas the pilots with smoke.

Oh Dear God,

It seems the fuel was cut off to the engines.  Maybe there was a good reason to do so.  Just hope it wasn't a nutter in the cockpit but that very well might be what happened.

May be something related to maintenence status/history of those switches and their interfaces with the fuel pumps.

Maybe but then again maybe the dude not driving decided "No fuel for you Buckoo" and flic'ed that Bic to the off position on the sly.

We'll see.

We've certainly seen commercial pilots purposely crash passenger filled airliners, so that possibility has to be considered.  I'd think turning off fuel pumps in flight would activate an annunciator light or warning signal of some kind, however.  In that case, you'd expect to hear the aircrew mention it on the CVR, and you'd expect to see the event depicted on the FDR.  Could be what the article is getting at.

Not sure if I hope it was a nutter or not.  Better than some sort of horrid issue with the 787 I guess.  The guy taking off should be super focused on the task at hand.  Might not notice the cut off when it happened.  I'd imagine the 787 certainly has a warning when that is done - if not Bitchin' Betty herself getting involved.
