Chances are that old lady was very difficult. Who knows, maybe the nurse is just being a ghetto bitch.  Protocol should have been two people grabbing her arms and restraining her with devices.

Old people who are belligerent in Japan get kicked out of the facility leaving the family to scramble to find a new facility or be stuck with the belligerent elderly person.

Japan also has a common care industry practice of having a maximum time that somebody can be there. Even if they are great and pay on time, most of these places will not keep somebody for more than a fixed amount of time.
