If she has lost some weight and is also not aging well, it is common to get some loose skin under the neck for some women, commonly known as turkey neck (and I am not being snarky here nor kidding). The shadows to either side of that skin flap only emphasize it. It's not flattering, but it doesn't look that unusual to me.
