Car accident.
Reminds me of a Canadian fellow who won a 'Ding' from Art's New Years prediction show. He predicted that a professional wrestler would pass. (I think that three had died in previous years, so it was a relatively safe prediction to make.) A year later he would score again by predicting that there would be a school shooting. Even back in those days, it was another safe-bet prediction to make.
